Directed Donor Program

The directed donor program is designed for situations in which the recipient of a semen sample is not a sexually intimate partner of the directed donor, and the directed donor is known to the recipient. Examples of directed donor situations include, but are not limited to, a recipient's who has chosen a particular sperm donor directed for her use only, a husband who is going through an assisted reproductive technology procedure such as IVF using a gestational carrier known to the husband,or a single man using a surrogate known to him. Because donors are required to collect samples for the directed donor program at the laboratory, we can only offer this service to those whose donor is in the Columbus, Ohio area, or is willing to travel to Columbus. We will work with out of town directed donors as best we can in completing the process in as few days as possible. We can also assist you in finding another laboratory in your area. Program Overview
Directed donors are required to undergo the same viral and sexually transmitted disease testing, physical, and medical health history review, as all anonymous donors. The 180-day quarantine period that is required for anonymous donors is recommended for directed donors, but is not an absolute requirement unless requested by your physician. Please check with your physician for his/her preference on this topic. Cryobiology has Agreements and Contracts that both the donor and the recipient, as well as the recipient's husband or partner (if applicable) and/or gestational carrier or surrogate are required to sign prior to beginning the program. However, all parties involved may feel more comfortable consulting a lawyer about additional documentation pertaining to issues that may arise in the future.

The first step in the directed donor program is a semen analysis and test freeze/ thaw performed on the potential donor. This testing is not a requirement to participate in the program, however, it is strongly recommended. This test will confirm that the potential directed donor has an adequate number of sperm cells and that the cells will survive freezing and thawing at an acceptable rate for a donor. Most recipients are interested in using their donor's sample for artificial insemination, however, any assisted reproductive technology procedure (IVF, ICSI, etc.) can be performed once the donor has passed all applicable testing, screening, and the quarantine (if applicable). The acceptability of sample quality and sperm cell survival rate is, therefore, dependent upon the procedure the recipient and the user physician have decided is appropriate.

Donor Requirements
Once it is established that the donor has an adequate sample quality, viral testing and screening is initiated. The potential donor must undergo the following before he can be accepted as a donor:

  1. Completion of a medical health history, provided by Cryobiology
  2. Physical- performed by the donor's family physician or our Medical Director
  3. Viral test panel- performed on blood samples
  4. Sexually transmitted disease screen- performed on urine
    Note: The donor's first semen sample must be collected within 7 days of viral and STD testing, but cannot be collected until test results are received and all other screening (physical and genetic interview) is complete. Therefore, multiple testing panels may be required, dependent on a number of factors.  
  5. Telephone interview with Cryobiology's genetic counselor
Note: directed donor must be 18- 40 years of age. Donors 40 or over are acceptable only if the recipient and the user physician agree and document agreement on Special Release documentation (see Program Costs). 

Results of all of the above will be sent to the user physician who will make the final decision in regards to the donor's final acceptance or rejection. As mentioned above, the donor cannot collect the first semen sample for potential use until all viral test results have been received. If all test results are acceptable, the donor can begin collecting semen samples. If the user physician must review information due to a positive or inconclusive test result or concern detected during the genetic interview or physical, the donor must wait to begin collecting semen samples, resulting in additional test panels that may be required.

180- day Quarantine or 7 day Testing?
Cryobiology can perform an eligibility determination on a directed donor using either the 180- day quarantine or the 7- day testing option. The user physician must determine which option is most suitable for the recipient and indicate that option on the referral or script.

180- day Quarantine
The 180- day quarantine is the same quarantine that specimens from anonymous donors must undergo. Viral and STD testing is performed prior to collecting the first semen sample (but still within 7 days of the first collection) and then is repeated 180 days AFTER the donor’s final semen collection. Semen samples are collected during the “window” of negative test results.  From a cost standpoint, there will be two testing panels required in this scenario.

Day 4 or 5- Collect first semen sample, IF test results are received and acceptable and physical and genetic interview are acceptable
Day 7 - Collect semen sample 2. Note this is the final day for collecting a semen sample using the test results of the first testing panel performed 7 days ago. If the donor cannot keep the appointment for semen collection, physician review is required, etc., another testing panel must be performed so that the first semen sample is collected within 7 days of testing. There is no testing/ time requirement for semen sample 2.

Day 223- Viral and STD testing required post- 180- day quarantine. Once all results are received and acceptable, all semen samples (1-7) are released for use.

The 180- day quarantine option is more flexible for the donor’s schedule and does require at least two testing panels- one at the beginning of the process and one at the end. Not all recipients will require this many semen samples collected.

7 day Testing
If the 180- day quarantine option is waived, all semen samples must be collected within 7 days of testing, including the first semen collection. However, subsequent semen samples can be collected 7 days prior to OR 7 days after testing. Below is an example calendar using the 7- day testing option. PLEASE NOTE THAT THE 7 DAY OPTION REQUIRES VERY SPECIFIC TIME COMMITMENTS FROM THE DIRECTED DONOR. IF THESE DATES ARE NOT FOLLOWED EXACTLY, SOME SEMEN SAMPLES MAY NOT QUALIFY FOR RELEASE AND USE.

If additional semen samples are needed, the donor cannot keep the appointment for semen collection, physician review is required, etc., another testing panel must be performed. However, semen samples can be collected 7 days prior to OR 7 days after testing.

           - Blood and urine collection for testing (blood and urine are an ABSOLUTE requirement on this date since semen samples have been collected 7 days prior. If testing is not performed on Day 16, semen sample 3 cannot be used!)
Day 18- Collect semen sample 7
Day 20- Collect semen sample 8
Day 23- Collect semen sample 9

This sample schedule would provide the most semen samples for the testing and cost. Again, two testing panels would be required. In addition, Special Release documentation (see Program Costs) is required when the 7 day option is used, resulting in additional costs as well. Not all recipients will require this many semen samples collected. However, this schedule shows how the 7- day testing requirement can be met.

The initial semen analysis and test freeze/thaw fee must be paid at the time of service. The deposit must be paid prior to the donor's testing appointment and is non-refundable if the user physician deems the donor unacceptable. If the donor is acceptable, the deposit will be applied to the balance of any charges accrued while the donor is collecting samples. Billing will occur periodically for those charges.

Additional fees may be required for ethnic- specific and result verification tests. The recipient is responsible for any additional fees.

Release of Samples

If shipping is required to deliver specimens to the user physician, regular shipping rates are applicable.

Prior to release of semen samples, Cryobiology will make a donor eligibility determination based on the directed donor’s testing and screening (medical history and physical) results. Semen samples from ineligible directed donors can be used for assisted reproductive technology procedures, provided Special Release documentation (see Program Costs) is completed. Special Release documentation will formally document informed consent from Cryobiology, Cryobiology’s Medical Director, the recipient, and the user physician concerning use of a semen sample from an ineligible directed donor.

Storage Options

Cryobiology, Inc. provides long term storage of anonymous donor seman samples for various situations. The most common situation is for recipients who have established a pregnancy and desire full genetic siblings in the future. Recipients purchase vials and put them into long term storage to ensure the same donor will be available when they are ready to attempt their next pregnancy.

Another situation for long term storage is when a recipient is currently using a donor, but the donor has a limited number of vials remaining, or because of his popularity, the recipient is not sure when the supply will become limited. In this case, the recipient may choose to purchase and store vials to ensure the donor is reserved for her exclusive use. This is especially common for ethnic- specific donors or for recipients who use an extensive donor selection process and want to avoid going through it again.

Long term storage is recommended in all situations where use of the same donor is desired at some point in the future. It is the only way the availability of a donor can be guaranteed.

Long Term Storage Program Overview
Because the number of vials available on any particular donor is a finite number, we always recommend long term storage to ensure the availability of that donor. Vials placed in long term storage must be purchased and the first year of storage paid in advance. Vials are then pulled from the general inventory and labeled with the recipient's name. Storage and billing information is generated and the vials are transferred to separate recipient storage. Fees for long term storage are billed on an annual basis (no pro- rating), with the billing date as the first date vials are put into storage. Additional vials can always be purchased and added to the recipient's inventory without additional storage fees, but the billing date for storage remains the original purchase date.

As long as vials are stored at Cryobiology, Inc. facilities, any vials put into long term storage can be returned provided they have NOT left our facility AND the donor is still available on our donor list. If the donor is no longer available, we will attempt to sell the vials by first consulting our waiting list for interested patients, and if unsuccessful, we will advertise the vials on our donor list. When someone else purchases the vials, the original recipient will then receive a refund of the original purchase price less a 30% per vial restocking fee.

Long Term Storage Waiting List
We maintain a waiting list for those recipients who did not put vials into long term storage but desire use of a particular donor. The waiting list is consulted when someone who has vials in storage is finished using them but the donor is no longer available on the regular donor list. We contact recipients in the order they were added to the waiting list and give each the option of purchase and storage at that time.

Why Use the Long Term Storage Program?
Because donors participate in the anonymous donor program for a limited period of time, when the vials they produced are all sold, there is no possibility of obtaining more. When our donors leave the program, they sign an agreement which states that they will not be contacted in the future for the purpose of producing additional vials. We honor that agreement.

Short Term Storage Program
Short term storage (maximum = 3 months) is available for recipients who need some extra time prior to their insemination: for example, those recipients who aren't ready to have vials shipped immediately but want to get the purchasing portion complete, for recipients who want to purchase multiple vials of a donor, but not have all of them shipped immediately, or for recipients who wants to ensure a 3 month supply of a donor remains available.

After the 3 month storage period, if vials remain at Cryobiology, options are as follows (the option chosen must be documented in writing):
Ship all remaining vials to another facility or to your physician.
Move all remaining vials to Long Term Storage (see above). The annual storage period begins when the vials are moved to long term.
Return all remaining vials to Cryobiology. The  purchase price per vial is refundable less a 30% restocking fee if the donor is still available on our donor list. If the donor is no longer available on our donor list, a refund (less the restocking fee) will not be given until we are able to re-distribute the vials.

Sperm Banking

General Information
We have three locations nationwide (see Fees below) that offer sperm banking to males wishing to preserve their future fertility.

Many men in the following situations choose to cryopreserve their sperm:

  • Chemotherapy, Radiation Therapy, a Medication Regimen, or other treatment that may decrease fertility
  • Testicular, Prostate, or other Urological Surgery
  • Vasectomy
  • Exposure to Chemical, Biological, or Environmental Hazards (herbicides, radiation, etc.) that may decrease fertility
  • Active Military Duty/Frequent Travel
  • Involvement in an ART procedure
  • Involvement in Sports Activities where there is an increased risk of injury to the reproductive system.

It is recommended that sperm samples be cryopreserved prior to treatment, surgery, and/or exposure to chemical, biological, or environmental hazards for the best quality sample.Potential future uses of cryopreserved specimens include IUI, IVF, ICSI, and other ART-related procedures. Please note that quality of the sample post thaw will most likely dictate which procedure(s) can potentially be used. See FAQ for details.

 

Scheduling an Appointment
In general, appointments are made Monday through Friday from 8:00am-2:00pm. However there are times when the timeframe or scheduling of other necessary procedures that you need takes a precedence. We make every effort to find a time to accomodate you and your needs.

Physician's Referral
All patients interested in sperm banking must be under the care of a physician and obtain a physician's prescription, or referral. All cryopreservation reports and viral testing results of the patient will be released to the referring physician only. The physician's referral should request: Sperm cryopreservation and viral testing necessary for banking. If more than one semen sample will be frozen and stored, the physician's referral must include the number of samples to be cryopreserved, or a reference to "multiple samples." The physician's referral may be faxed to our office prior to the scheduled appointment or delivered by the patient during the first scheduled appointment.

Viral Disease Testing
All patients will be screened for the following viral diseases during the first scheduled appointment: HIV 1 & 2, Hepatitis B surface Antigen, and Hepatitis C testing.

Recommended Abstinence
For the best quality sample, we recommend 2-5 days abstinence from any ejaculation prior to your appointment. Any semen sample submitted, regardless of days abstinence, can be cryopreserved. However, please be aware that the sample quality may not be optimal. We are aware that some medical treatment regimens, or timeframes, may not allow the recommended days of abstinence. Remember that this is only a recommendation, NOT a requirement to cryopreserve a sample.
